Under the rule of the military junta, Myanmar's western region, rich in resources, has been coveted by the military for many years. Children there are often forcibly recruited by the army, and massacres of civilians have become a form of entertainment for some soldiers... A volunteer team from a church trying to enter the area to provide humanitarian aid to refugees finds the reclusive Rambo (played by Sylvester Stallone) in Thailand, hoping he can help them navigate the waterways. Rambo believes the situation is unlikely to change and refuses to guide them but is eventually moved by the sincerity of volunteer Sarah (played by Julie Benz) and agrees to take them there. Unfortunately, the volunteer team is attacked by the military in a village, and all survivors are captured. The church learns about the volunteers' perilous situation and organizes mercenaries for a rescue, once again entrusting Rambo to lead the way. This time, Rambo's attitude changes, and he leads the mercenaries into a Myanmar military camp, once again becoming a battle-hardened warrior.
