Five American amateur wrestlers, Alex (played by Thora Birch), Todd, Claire, Willy, and Shelton, under the guidance of their coach, traveled to Eastern Europe to participate in a wrestling competition. Although they lost the match, they enthusiastically attended a party that night, which caused them to miss their train to Odessa the next day. Due to a language barrier, they were initially unable to buy tickets, until a blonde woman who spoke English appeared and helped them board a red train at the station.Upon boarding, the team members had their passports taken by a sleazy-looking male conductor. The modified train was equipped with a casino, dining car, and lounge, and the team quickly distanced themselves from other non-English-speaking passengers, hiding in the lounge to play Truth or Dare. However, during the game, Todd disappeared towards the rear of the train, and the other team members began to go missing one by one. Alex soon realized she had to face several organ harvesters on the train all alone...
