Adam (played by Ben Whishaw) is an obstetrician working in a public hospital, where the medical equipment and personnel allocation are far inferior to those in private hospitals. Here, Adam not only has to face endless overtime but also has to engage in a battle of wits with difficult and demanding patients, living under constant immense pressure. Fortunately, Adam has made some friends at the hospital, such as the novice intern Shudi (played by Ambika Mod). Adam has a boyfriend, Harry (played by Rory Fleck Byrne), with whom he has been in a relationship for many years. Although their relationship is stable, Adam has yet to reveal his sexual orientation to his parents, let alone introduce Harry to them. An accident leads to Adam's misdiagnosis, causing a baby to be born prematurely. This incident becomes a tightening noose around his neck, gradually suffocating him.
