In the third installment, Dieter Laser, who played the sadistic doctor in the first film, and Laurence R. Harvey, who portrayed the deranged fat man in the second film, will gather together. The director, Hicks, wants to take the meta aspect to the extreme by playing himself in the third part. Additionally, 'The Human Centipede 3' will also introduce a new antagonist. In 2009, the character played by Dieter Laser, the sinister mastermind who initiated the three-person conjoined experiment, was introduced. The second film, 'The Human Centipede 2', was released amid controversy, and now both villains will come together in the third film, which also features the director himself, so the level of grotesquery is to be expected. Laser plays a violent warden whose prison has the highest rates of inmate riots, medical expenses, and transfers of guards in the country. More crucially, he believes that these inmates and Governor (played by Roberts) do not give him the respect he deserves. He has tried many punitive measures but still cannot manage to keep order, and the governor has given him an ultimatum: either manage the inmates or shut down the prison. Following the advice of his right-hand man Dwight (played by Laurence), he comes up with an extremely 'creative' idea that will allow him to control the inmates while saving millions of dollars, and this idea comes directly from the 'Human Centipede' movie!
