The mastermind behind the organ trafficking group, Hong Wengang (played by Louis Koo), is suffering from severe heart disease. To survive, he even plans to take the heart of his younger brother, Hong Wenbiao (played by Gordon Lam). In order to complete his undercover work, Zhi Jie (played by Wu Jing) becomes addicted to drugs and accompanies his boss to the docks to kidnap Hong Wenbiao, who attempts to escape. When his superior, Uncle Chen Guohua (played by Sammo Hung), receives a tip-off, he leads a team to the scene, and a fierce gunfight ensues among the three parties. Zhi Jie, who narrowly escapes, is sent to a prison in Thailand, where the warden (played by Zhang Jin) has intricate connections with Hong Wengang. A prison guard, A Chai (played by Tony Jaa), whose daughter suffers from leukemia, grows suspicious of Zhi Jie’s true identity, and later inadvertently discovers a terrifying secret within the prison. To rescue his nephew, Chen Guohua travels all the way to Thailand. Meanwhile, Hong Wengang is still desperately struggling, ultimately descending into madness.
